CareBrains LLC delivers specialized care management programs designed to improve outcomes for patients with chronic conditions.
The ACCESS Model Program
The CMS ACCESS (Advancing Chronic Care with Effective Scalable Solutions) Model program is a Centers for Medicare & Medicaid Services initiative focused on improving care coordination and health outcomes for Medicare beneficiaries living with chronic conditions.
CareBrains LLC partners with Medicare beneficiaries to provide regular, technology-enhanced services under the ACCESS Model to address depression and anxiety, especially in family caregivers who support people living with dementia.
The GUIDE Model Program
The CMS GUIDE Model (Guiding an Improved Dementia Experience) is an 8-year nationwide Medicare initiative launched on July 1, 2024, designed to improve comprehensive care and support for people with dementia and their unpaid caregivers.
Monthly Care Navigation Services are provided at no charge to eligible Medicare beneficiaries. Some beneficiaries also may be eligible for $2600 worth of respite care provided by vetted agencies.
Care Coordination & Management
Comprehensive care coordination services for Medicare beneficiaries with multiple chronic conditions, including care plan development, transitions of care, and ongoing monitoring. CareBrains also participates in the CMS GUIDE model serving people living with dementia.
ACCESS Model
End-to-end support for enrolling and participating in the CMS ACCESS Model. Regular technology-supported education, outreach, and referrals to aid Medicare beneficiaries. There is no fee to participate if you are enrolled in traditional Medicare.
GUIDE Model
Comprehensive dementia care navigation under the CMS GUIDE Model (Guiding an Improved Dementia Experience). Monthly Care Navigation Services at no charge to eligible Medicare beneficiaries, plus up to $2,600 in respite care through vetted agencies for qualifying beneficiaries.
Chronic Disease Management
Evidence-based programs targeting high-prevalence chronic conditions including anxiety and depression to reduce hospitalizations and improve quality of life.
Caregiver & Family Support
Education, resources, and coordination services for family caregivers, empowering them to actively participate in their loved one's care journey.
