A dedicated care management organization committed to improving the lives of Medicare beneficiaries living with chronic conditions.

Our Mission

CareBrains LLC was founded with a singular purpose: to bridge the gap between Medicare beneficiaries with chronic conditions and the coordinated, compassionate care they deserve.

Through our participation in the CMS ACCESS Model and GUIDE Model programs, we work alongside patients, families, and healthcare providers to deliver evidence-based care management that reduces hospitalizations, supports caregivers, and improves quality of life.

Patient-Centered Care

Every decision we make starts with the patient. We design our programs around the unique needs, goals, and circumstances of each individual we serve.

Caregiver Support

We recognize the vital role family caregivers play. Our programs provide education, resources, and emotional support to those caring for loved ones with dementia and other chronic conditions.

Our care management protocols are grounded in clinical evidence and aligned with CMS quality standards to ensure the best possible outcomes.

We collaborate with healthcare providers, community organizations, and payers to build a connected network of support around every patient we serve.

Our Programs

CareBrains LLC participates in the CMS ACCESS (Advancing Chronic Care with Effective Scalable Solutions) Model, focused on improving care coordination for Medicare beneficiaries with chronic conditions including anxiety and depression. The ACCESS Model (Advancing Chronic Care with Effective, Scalable Solutions) is a 10-year Centers for Medicare & Medicaid Services (CMS) initiative launched in July 2026 that uses outcome-based payments to expand technology-supported chronic disease care for Original Medicare beneficiaries

We also participate in the CMS GUIDE (Guiding an Improved Dementia Experience) Model, serving people living with dementia and their family caregivers through specialized care navigation and support services. The CMS GUIDE Model (Guiding an Improved Dementia Experience) is an 8-year nationwide Medicare initiative launched on July 1, 2024, designed to improve comprehensive care and support for people with dementia and their unpaid caregivers
